Sisomicin ist ein Aminoglykosid-Antibiotikum, das hauptsächlich zur Behandlung von Infektionen eingesetzt wird, die durch gramnegative Bakterien verursacht werden. Es wird aus der Fermentation des Bakteriums Micromonospora purpurea gewonnen. Sisomicin zeigt ein breites Spektrum antibakterieller Aktivität und ist besonders wirksam gegen verschiedene Stämme von Pseudomonas und Enterobacter-Arten. Der Wirkmechanismus besteht darin, an die 30S-ribosomale Untereinheit zu binden, was zur Hemmung der Proteinsynthese führt und letztendlich zum Absterben der Bakterienzellen führt. Sisomicin wird typischerweise durch Injektion verabreicht, da es eine schlechte orale Bioverfügbarkeit aufweist. Seine Pharmakokinetik umfasst die renale Ausscheidung, was Dosisanpassungen bei Patienten mit eingeschränkter Nierenfunktion erforderlich macht. Obwohl es im Allgemeinen gut vertragen wird, können potenzielle Nebenwirkungen Nierenschäden und Ohrenschäden umfassen, häufige Bedenken, die mit Aminoglykosiden verbunden sind. Die chemische Struktur von Sisomicin weist mehrere Aminogruppen auf, die zu seiner Löslichkeit und Interaktion mit bakteriellen Ribosomen beitragen.
