Pamidronat-Dinatrium ist eine Bisphosphonatverbindung, die hauptsächlich zur Behandlung von Erkrankungen eingesetzt wird, die durch übermäßigen Knochenabbau gekennzeichnet sind, wie Osteoporose und Morbus Paget, sowie zur Behandlung von Hyperkalzämie im Zusammenhang mit Malignitäten. Es wirkt, indem es die durch Osteoklasten vermittelte Knochenresorption hemmt und somit hilft, die Knochendichte und -stärke aufrechtzuerhalten. Die Substanz wird typischerweise intravenös verabreicht, da sie eine geringe orale Bioverfügbarkeit aufweist. Pamidronat-Dinatrium ist ein weißes bis schwach weißes Pulver, das in Wasser löslich ist, was es für parenterale Formulierungen geeignet macht. Seine chemische Struktur weist zwei Phosphonatgruppen auf, die für seine hohe Affinität zu Knochengewebe verantwortlich sind. Die Verbindung wird im Allgemeinen gut vertragen, kann jedoch Nebenwirkungen wie grippeähnliche Symptome, gastrointestinale Störungen und in seltenen Fällen Osteonekrose des Kiefers verursachen. Wie bei anderen Bisphosphonaten ist es wichtig, die Nierenfunktion während der Behandlung zu überwachen, da das Medikament hauptsächlich über die Nieren ausgeschieden wird.
