Pregnanetriolone is an intermediate in the synthesis of progesterone. It is also a substrate for enzymes that catalyze dehydrogenation reactions, such as adrenocortical carcinoma. Pregnanetriolone has been found to be elevated in the serum and urine of patients with adrenocortical carcinoma and infantile adrenal hyperplasia. The enzyme deficiency can be diagnosed by measuring the concentration of pregna-5α-triol in urine. Pregnanetriolone is unique among steroids in that it is not readily oxidized at neutral pH and may be used as a diagnostic marker for these conditions. Treatment consists of specific therapy, such as surgery or chemotherapy, depending on the severity of the condition.
