Clozapine, a dibenzodiazepine, demonstrates antagonistic activity at the dopamine receptors, further binding to serotonin (5-HT1A, 5-HT2A-C, 5-HT6, 5-HT7), alpha 1 and 2 adrenergic, histamine 1 and M1-M5 muscarinic receptors. In particular, its partial 5-HT1A agonist behaviour, its low affinity for the D2 Dopamine receptors and its antagonistic activity at 5-HT2A receptors, are properties, which make it useful as an antipsychotic drug. The FDA has approved Clozapine for use in the alleviation of symptoms in treatment-resistant schizophrenia patients, such as the risk of self-harm. Although not yet approved by the FDA, Clozapione exerts positive effects in Parkinson’s disease and bipolar disorder treatments. However, Clozapione is not currently a first line-therapy option, as it may cause agranulocytosis and other side effects such as orthostatic hypotension and hypersalivation.
