Apcitentan is a vasoactive drug that belongs to the group of endothelin receptor antagonists. It is used in the treatment of erectile dysfunction and pulmonary hypertension. Apcitentan inhibits the angiotensin system, which includes angiotensin-converting enzyme (ACE), angiotensin II type 1 receptor (AT1R) and angiotensin II type 2 receptor (AT2R). This inhibition increases the levels of endothelin and dopamine β-hydroxylase, thereby decreasing blood pressure. Apcitentan has been shown to increase glomerular filtration rate and reduce proteinuria in patients with chronic kidney disease. In vivo studies have shown that apcitentan is rapidly absorbed from the gastrointestinal tract, reaching peak concentrations within one hour.
