4-Carboxy tolbutamide is a drug that inhibits the activity of the enzyme adenosine deaminase (ADA), which converts adenosine to inosine. It is used for the treatment of type 1 diabetes mellitus, but is not effective for type 2. 4-Carboxy tolbutamide has been shown to have an inhibitory effect on polymerase chain reaction and recombinant cytochrome P450 metabolizing enzymes, as well as on human erythrocytes and liver microsomes. The hypoglycemic effect of 4-carboxy tolbutamide may be due to its ability to increase cytosolic calcium concentrations. This drug has also been shown to have a concentration-dependent time curve in vitro, indicating that it may be metabolized by cytochrome P450 enzymes in vivo.
