9-epi-quinine is a drug that is used to treat and prevent malaria. It is an analog of quinine and has been shown to have similar effects in inhibiting the growth of malaria parasites. 9-epi-quinine binds to the polymerase chain reaction (PCR) product, which prevents the formation of a covalent bond between DNA and polymerase during replication. This leads to inhibition of DNA synthesis and cell death. 9-epi-quinine also inhibits cardiac enzyme activities, such as p-nitrophenyl phosphate oxidase, which leads to decreased production of nitric oxide. The interaction between 9-epi-quinine and quinidine, a drug used for treating heart conditions such as atrial fibrillation, may lead to toxicity when taken together. 9-epi-quinine has been shown to cause congestive heart failure in mice by decreasing blood flow through the coronary artery.
