Finasteride is a carboxamide that is metabolized by the liver to its active form, finasteride. It has been shown to inhibit an enzyme in the microsomal metabolic reaction of the liver, which catalyzes the conversion of testosterone to 5α-dihydrotestosterone (DHT). This inhibition leads to a decrease in DHT levels, resulting in decreased prostate size and symptoms associated with benign prostatic hyperplasia. Finasteride is also known as a potent inhibitor of 3-hydroxysteroid dehydrogenase (3β-HSD), which converts progesterone and 17α-hydroxypregnenolone into their corresponding 3-keto steroids.
