Flumazenil is an antagonist of benzodiazepine drugs that has been used in the treatment of insomnia, as a premedication prior to anaesthesia and surgery, and as an antidote to benzodiazepines. Flumazenil is also used to reverse the effects of benzodiazepines in animals. Flumazenil binds competitively to the benzodiazepine site on the GABA receptor, preventing binding by GABA or other agonists such as zolpidem and zopiclone. In humans, flumazenil has been shown to be able to inhibit insulin secretion after intravenous administration. Its clinical use is limited because it produces withdrawal symptoms upon abrupt discontinuation. The drug was initially developed for use in industrial settings where there may be exposure to chlorinated hydrocarbons, which are known inhibitors of insulin secretion. This study showed that flumazenil could be used for this purpose, although it is not yet approved for this indication.
