It has been approved for use in postmenopausal women with estrogen receptor (ER) positive breast cancer that has progressed after tamoxifen therapy. 6-Hydroxymethyl exemestane inhibits the production of estrogen by inhibiting the enzyme aromatase, which is responsible for converting androgens to estrogens. This inhibition leads to decreased tumor progression and development. 6-Hydroxymethyl exemestane binds to the heme group at the active site of aromatase, thereby blocking its activity and preventing conversion of androgens to estrogens. This prevents the synthesis of estrogen, which may be responsible for breast cancer growth.
