The octacosapeptide somatostatin-28 arises from the posttranslational cleavage of prosomatostatin, which in turn is derived from a larger precursor, preprosomatostatin. S-28 was first isolated from porcine duodenum. In guinea-pig pancreatic acini, S-28, unlike S-14, increased amylase secretion, cellular cyclic GMP, and Ca²⁺ outfluxes.

It has been found to inhibit pituitary growth hormone release as well as endocrine, pancreatic and GI secretions. In the central nervous system somatostatin plays a role in neurotransmission modification and it has also demonstrated to be effective against a number of cancers such as squamous carcinoma.
