21-Deoxydexamethasone is a synthetic corticosteroid that has been extracted from the adrenal cortex of animals. It is hydrolysed to deoxycorticosterone, which is then converted to corticosterone in the liver. The 21-deoxysteroid and its derivatives are used as a source of cortisone, while the 21-hydroxylated compounds are used in some synthetic glucocorticoids. Deoxycorticosterone can also be converted into hydrocortisone by β-glucuronidase. It has been shown that this drug has a high urinary excretion rate (approximately 50%), and has been analysed using spectrometry to identify conjugates. Chemical ionisation was used for identification of metabolites in urine samples, with detection limits of 0.1 ng/mL for metabolites such as 17α-dehydrodeoxycortisol and 17α-hydroxydeoxycortisol
