Midecamycin is a macrolide antibiotic that is a covalent inhibitor of the ryanodine receptor, which regulates calcium release in muscle cells. Midecamycin has been shown to have antimicrobial activity against infectious diseases and bowel disease. It also inhibits the release of IL-2 in response to IL-2 receptor binding, which may be due to its ability to inhibit the enzyme activities of protein kinase C and phosphodiesterase. This drug also causes liver lesions, as well as enzyme inactivation. Midecamycin is not metabolized by cytochrome P450 enzymes, but does inhibit CYP3A4 and CYP2D6. Midecamycin interacts with oral cephalosporins such as cefuroxime axetil, cefpodoxime proxetil, and ceftibuten.
