Alcaftadine is a selective histamine H1 receptor antagonist. It inhibits the action of histamine on its receptor, thereby reducing the symptoms of an allergic reaction. Alcaftadine is metabolized by hydrolysis to an acylated derivative which undergoes an acylation reaction with a nucleophilic amino acid in the presence of hydrochloric acid. The drug interacts with other drugs that are metabolized by cytochrome P450 3A4, such as benzodiazepines and anticonvulsants. Alcaftadine has a long half-life, ranging from 16 to 30 hours, and is eliminated in urine unchanged or as metabolites. It also has pharmacokinetic properties that may be affected by polymorphism at the 2-adrenergic receptor gene locus (DRD2).
