Diltiazem hydrochloride is a calcium channel blocker that slows the flow of calcium into cardiac and smooth muscle cells. It is used to treat high blood pressure and angina. Diltiazem hydrochloride reversibly binds to the protein glycoprotein IIb/IIIa (GPIIb/IIIa), which is found on platelets and vascular endothelium, thereby inhibiting platelet aggregation. Its anti-anginal effects are attributed to its ability to inhibit the release of acetylcholine and other neurotransmitters from nerve endings in the heart. Diltiazem hydrochloride inhibits myocardial infarction by reducing oxygen demand in patients with coronary artery disease. This drug also slows down the heart rate and reduces blood pressure by blocking the influx of sodium ions into cardiac cells, which may cause an increase in intracellular calcium ions levels.
