Hexamidine diisethionate works by reacting with the nitrogen atoms of organic molecules to form nitrosamines, which are reactive and highly cytotoxic. These nitrosamines inhibit the DNA polymerase of human pathogens and other microorganisms, preventing their growth. They also prevent glycol esters from being hydrolyzed to glycols, which would cause them to be toxic themselves.
