Haldol is a neuroleptic drug that blocks dopamine receptors and has a calming effect in people with schizophrenia. It is also used in the treatment of severe psychotic disorders, such as manic-depressive illness, delusional
mania, and other psychotic conditions. Haldol is metabolized by conjugation with glucuronic acid to form 7-hydroxy fluphenazine glucuronide. This active form binds to dopamine receptors on the postsynaptic membrane of neurons, blocking the binding of dopamine. The clinical response to Haldol may be due to its ability to block dopamine receptors at different levels: it may inhibit the release of dopamine from nerve endings and prevent reuptake into nerve endings. Haldol also binds with high affinity to monoclonal antibodies against D2 receptor sites, which are found in high concentrations in the human brain. This binding prevents dopamine from binding and blocking the receptor site on the postsynaptic membrane.
