Tetrahydrobiopterin (BH4) is a cofactor of the enzyme phenylalanine hydroxylase, which converts phenylalanine to tyrosine. BH4 is also an important cofactor for nitric oxide synthase, which catalyzes the conversion of arginine and oxygen to citrulline and nitric oxide. It has been shown that BH4 can protect cells against oxidative injury by scavenging reactive oxygen species. The synthesis of tetrahydrobiopterin can be blocked by sulfa drugs, such as sulfamethoxazole and sulfasalazine, which are used in the treatment of inflammatory bowel disease. Tetrahydrobiopterin has been shown to have antioxidant properties, which may be due to its ability to act as a free radical scavenger or chelator of metal ions.
