Phenobarbital is a barbiturate that has been used as an anticonvulsant and sedative since the 1920s. It is metabolized by the liver cytochrome P450 enzyme CYP1A2, which converts it to its active form, phenobarbital-10,11-epoxide. Phenobarbital is excreted in the urine unchanged or as glucuronide and sulfate conjugates. The average age of people taking phenobarbital is 47 years old. The average total daily dose of phenobarbital is 1.8 mg/kg body weight (or 178 mg). The drug has a low risk of toxicity because it is not very toxic and has a low therapeutic index. Phenobarbital also increases the level of cytochrome P450 enzymes in the liver, which can lead to drug interactions with other drugs metabolized by these enzymes.
