Sorbinil is a potent inhibitor of sorbitol dehydrogenase. It has been shown to reduce the blood glucose level in diabetic rats. Sorbinil binds to the enzyme and prevents it from catalyzing the conversion of sorbitol to fructose, which results in an accumulation of sorbitol in cells and tissues. This accumulation reduces the production of glucose from glycogen by inhibiting enzymes such as alpha-glucosidase and alpha-amylase. Sorbinil also inhibits reductases, which are enzymes that reduce various substances. The nonsteroidal anti-inflammatory drug diclofenac has been found to inhibit reductases and may be used as an adjuvant treatment for diabetes.
Sorbinil also has a strong affinity for DNA binding activity, particularly with respect to cytosine residues. It is a competitive inhibitor of type II deoxyribonucleotide reductase, which catalyzes the reduction of ribose-5
