It is used as a progestin in combination with an estrogen for hormonal contraception, and as an antifungal agent in the treatment of vaginal candidiasis. Ethynodiol diacetate has been shown to have a genotoxic activity, which may be due to its ability to inhibit DNA synthesis and repair. It also inhibits protein synthesis in bacteria by binding to 16S ribosomal RNA.
