Carbamazepine binds voltage-gated sodium channels when in their inactive conformation, preventing repetitive and sustained firing. This interaction reduces the frequency and duration of sodium channel opening, slowing down nerve impulses and helping to prevent seizures. As sodium channels are involved in the initiation and propagation of nerve impulses in the brain, they play a key role in the development of seizures. By binding to these channels, carbamazepine stabilizes the electrical activity in the brain and reduces the likelihood of seizures. Carbamazepine is also a GABA agonist as well as having effects on calcium channels and serotonin systems. This leads to further stabilization of the electrical activity in the brain.
