Boc-trans-4-hydroxy-D-proline is a peptidase inhibitor that has been shown to be effective in the treatment of diabetes. It interacts with the enzyme dipeptidyl peptidase 4 (DPP-4) by binding to the active site subsite where it competitively inhibits enzymatic activity. This inhibition prevents the breakdown of incretin hormones, which are substances that increase insulin secretion and reduce glucagon levels. The oral glucose tolerance test showed that Boc-trans-4-hydroxy-D-proline decreased plasma glucose levels in rats with diabetes, as well as reduced blood glucose levels in patients with type 2 diabetes. Boc-trans-4-hydroxy-D proline may also have antihypertensive effects due to its ability to decrease plasma glucose levels.
