Diammonium 2-(chloromethyl)-5-methylpyrazine hydrochloride is a diammonium salt of 5-methylpyrazine. It is used as an antidote for acetaminophen overdose, but only when the patient has not developed liver damage. Diammonium 2-(chloromethyl)-5-methylpyrazine hydrochloride binds to acetaminophen and prevents it from binding with liver enzymes that catalyze the conversion of acetaminophen to an active metabolite. This prevents the damaging effects of acetaminophen on the liver, which may be due to its ability to inhibit hepatic microsomal enzyme activity.
