(+)-Norphenylephrine hydrochloride is a crystalline cellulose that is used in the treatment of hypotension, orthostatic hypotension, and postural tachycardia syndrome. It has been shown to increase blood pressure by acting on the detrusor muscle of the bladder. It has also been shown to increase urine flow by stimulating the release of dopamine from nerve terminals in the brain. This neurotransmitter acts on dopamine β-hydroxylase, which converts dopamine to norepinephrine. The conversion of dopamine to norepinephrine increases urinary output and decreases heart rate and cardiac output. Pharmacological doses of (+)-norphenylephrine hydrochloride have not been shown to affect blood levels of dopamine or other monoamine neurotransmitters. However, at physiological levels, it can inhibit DNA methyltransferases and reduce the synthesis of dopamine from tyrosine.
