Butylidenephthalide is a mixture of the cis and trans isomers. The cis-trans ratio in butylidene phthalide varies with the source material and can range from 0.1 to 1.0. It has been shown that the cis form has a higher level of neurotoxicity than the trans form, which is thought to be due to an increase in the mitochondrial membrane potential.
