(S)-Acenocoumarol is a drug that is metabolized by cytochrome P450 2C19 to form the active metabolite (R)-acenocoumarol. This drug is used in the prevention of clotting and stroke in people with atrial fibrillation or those who have had a transient ischemic attack. The pharmacological effect of this drug is mediated through its ability to inhibit the conversion of vitamin K to its active form, which leads to decreased production of prothrombin. Oral administration of (S)-acenocoumarol has been shown to increase blood levels in human liver biopsies due to its high uptake into this organ. Pharmacodynamics experiments indicate that constant doses of (S)-acenocoumarol produce dose-dependent pharmacological effects on human liver tissue.
