2-(3-Pyridyl)-2-oxazoline is a nicotinic acid derivative that has been shown to have vasodilatory effects on the coronary artery. This compound is activated by nitrate, which causes the ring-opening of the oxazoline ring to form a 2-pyridinecarboxylic acid. Nicorandil is metabolized by plasma esterases and hydrolyzed by plasma carboxylesterase to release nicotinamide, which then acts as an inhibitor of phosphodiesterase. Nicorandil does not affect platelets or blood pressure, but it does reduce the risk of death in patients with heart failure. It also has a low level of side effects and has been shown to be effective at preventing angina pectoris and myocardial infarction in patients with coronary artery disease.
