Octahydro-1H-isoindol-4-one is a potent opioid analgesic drug that binds to the opioid receptors in the central nervous system and blocks pain. It is an enantiomer of racemic (1:1) naloxone, with about 10 times the affinity for these receptors and potency when compared with morphine. Octahydro-1H-isoindol-4-one has been shown to be effective in treating chronic pain and hyperreflexia in animals. The mechanism of action may be due to its ability to inhibit voltage-dependent Ca2+ channels, thereby reducing neurotransmitter release from sensory nerves and blocking nociceptive transmission at the spinal cord level. In addition, it inhibits ileum contractions, which may be due to its effect on Ca2+ channels in smooth muscle cells.
