Erythromycin is a macrolide antibiotic that is used to treat bacterial infections. Erythromycin iactobionate (EIB) is an ester prodrug of erythromycin. It has been shown to be effective against cardiac, papillary muscle, and infectious diseases. EIB has been shown to have chemical stability with clinically relevant concentrations of erythromycin and its metabolites in the serum after intravenous administration. The reaction mechanism of erythromycin may involve inhibition of protein synthesis by binding to ribosomes at the 50S site, which blocks translocation, or by inhibition of bacterial DNA gyrase and topoisomerase IV. Symptoms of erythromycin toxicity include gastrointestinal disturbances such as nausea, vomiting, and diarrhea; neurological manifestations such as seizures; hepatic dysfunction such as elevated liver enzymes; and cardiac arrhythmias such as tachycardia and bradycardia.
