Guanfacine is a drug that is used to treat high blood pressure, anxiety disorders, and attention-deficit hyperactivity disorder. It is an alpha2A receptor agonist and thus it has antihypertensive, antidepressant, and antipsychotic effects. Guanfacine can be metabolized through a number of metabolic transformations, including sulfation and glucuronidation. The methylation of guanfacine by catechol-O-methyltransferase (COMT) produces the active form of the drug, 3-hydroxy guanfacine. This active form can then be metabolized through monooxidation or enzymatic hydrolysis to produce metabolites that are more polar than the parent compound. Guanfacine is also an antagonist at postsynaptic alpha2B receptors and it has been shown to inhibit catecholamine synthesis in animals.
