α-Hydroxy trimethoprim (impurity) is a nonsteroidal anti-inflammatory drug that has been used for the treatment of rheumatoid arthritis, osteoarthritis, and ankylosing spondylitis. It is metabolized in the liver to produce a carboxylic acid metabolite with anti-inflammatory properties. The hydroxyl group on α-hydroxy trimethoprim can be converted to an amide, which can be transformed into enalaprilat. This impurity also inhibits the production of glucose by cells in the pancreas and helps to control blood sugar levels in patients with diabetes mellitus type 2. The structure of α-hydroxy trimethoprim may also have reactivity with glutathione or cytochrome P450 enzymes.
