Cilomilast is a non-selective phosphodiesterase inhibitor that acts on cyclic nucleotide phosphodiesterases and blocks the degradation of cyclic adenosine monophosphate (cAMP) and cyclic guanosine monophosphate (cGMP). It has been shown to inhibit mucin gene expression in bowel disease models, reduce collagen gel contraction, and inhibit the binding of an experimental model for cilomilast to human erythrocytes. Cilomilast also has anti-inflammatory activity in vivo. Cilomilast is not metabolized by cytochrome P450 enzymes, so it does not affect hepatic metabolism.
