Cefcapene pivoxil hydrochloride monohydrate (CPHM) is a prodrug that is converted to its active form, pivoxil, in the body. The conversion of CPHM to pivoxil is catalyzed by diisopropylamine and hydrochloric acid in water. CPHM is not water-soluble but can be converted into a water-soluble drug by esterification with trifluoroacetic acid. This reaction has been shown to occur in vitro and can be expressed as a plasmid construct in Escherichia coli. CPHM has antimicrobial activity against Gram-positive bacteria, such as Staphylococcus aureus, and Gram-negative bacteria, such as Escherichia coli. CPHM has pharmacokinetic properties that are similar to pivoxil.
