Rilpivirine is an antiviral drug that is used in the treatment of HIV. It inhibits the viral enzyme reverse transcriptase, which is needed for the production of viral DNA from RNA. Rilpivirine is a prodrug that undergoes hydrolysis by esterases to form its active metabolite, 4-[(4-hydroxy-2-pyrimidinyl)amino]benzonitrile (4HPBA). 4HPBA binds to reverse transcriptase and prevents DNA synthesis. Rilpivirine has been shown to be active against HIV strains resistant to other anti-HIV drugs such as zidovudine, abacavir and lamivudine.
