Tetrahydrobiopterin is a cofactor in the synthesis of nitric oxide, which is important in maintaining the integrity of the endothelium. It also plays a role in the regulation of blood pressure and postprandial glucose levels. Tetrahydrobiopterin is usually produced by the reduction of L-erythro-biopterin, which occurs through an enzymatic reaction with dihydrochloride reductase. This process can be inhibited by oxidative stress, which prevents the reduction of L-erythro-biopterin to tetrahydrobiopterin. In vitro studies have shown that oxidative stress inhibits nitroglycerine-induced vasodilation, suggesting that tetrahydrobiopterin may play a role in endothelial dysfunction. Supplementation with oral glucose or sulfate has been shown to increase blood flow and improve endothelial function.
