5-Acetyl-2-methoxybenzonitrile is an aldehyde that has been shown to be metabolically stable, but is covalently bound to the hemoglobin. It causes sickling by forming crosslinks between the chains of the red blood cells that cannot be broken down. 5-Acetyl-2-methoxybenzonitrile has affinity for the alpha and beta chains of hemoglobin, which are believed to be due to its similarity in structure to acetaldehyde and formaldehyde. This affinity leads to a competitive inhibition of binding between oxygen molecules and hemoglobin molecules. The hypoxia induced by this binding leads to clinical modifications in patients, such as long acting effects, pathological changes, and pharmacologic responses.
