Azilsartan D5 is a type 1 receptor blocker that is used for the treatment of high blood pressure. Azilsartan D5 is a prodrug, which is metabolized to its active form azilsartan by esterases in the blood and liver. The mechanism of action of this drug is to inhibit angiotensin I-converting enzyme, which reduces the levels of angiotensin II (a potent vasoconstrictor) and lowers blood pressure. Azilsartan D5 also blocks the binding of angiotensin II to its type 1 receptor. The methyl group on azilsartan D5 may be responsible for its ability to inhibit angiotensin I-converting enzyme and decrease serum aminotransferase levels. Medoxomil, an antagonist that blocks the effects of angiotensin II, has shown similar effects on lowering blood pressure in clinical trials but has not been approved for use in humans yet.
