Sodium DL-mandelate is a crystalline compound that has been shown to have bactericidal effects on Gram-positive and Gram-negative bacteria. It is also known to be effective against Mycobacterium tuberculosis and methicillin-resistant Staphylococcus aureus (MRSA) in vitro. Sodium DL-mandelate binds to the bacterial cell membrane, causing leakage of cellular contents, which leads to cell death. The mechanism of action is thought to be the formation of a toxic carbonyl group from the oxidation of mandelic acid by hydrochloric acid.
Sodium DL-mandelate has been used as an osmotic diuretic drug because it causes increased sodium excretion through urine. This drug also has been shown to increase insulin levels in humans and rats with type 2 diabetes mellitus, due to its ability to increase glucose uptake by cells and decrease hepatic gluconeogenesis.
