Fidaxomicin is a structural analog of vancomycin, which is a glycopeptide antibiotic. It is the first member of the glycylcyclines class of antibiotics to be discovered and developed for clinical use. Fidaxomicin has been shown to have excellent antibacterial activity against most Gram-positive bacteria and some Gram-negative bacteria. It inhibits bacterial growth by binding to the D-alanyl-D-alanine moiety in peptidoglycan precursors in bacterial cell walls. Fidaxomicin also binds to carbohydrate in the cell wall, which alters its catalytic properties. This drug has been shown to have a number of pharmacokinetic properties including high oral bioavailability and long half-life, making it an excellent candidate for treatment of tuberculosis.
