Bedaquiline is a new drug for the treatment of drug-resistant tuberculosis. It is a prodrug that is converted to its active form in the body by esterases and glucuronidases. Bedaquiline inhibits bacterial growth by binding to DNA-dependent RNA polymerase, thereby preventing transcription and replication. Bedaquiline has been evaluated in clinical trials with plasma concentrations of up to 12μg/L and was shown to be effective against resistant strains of tuberculosis (TB). Bedaquiline has been shown to have a pharmacokinetic profile that improves the efficacy of ethambutol, an antibiotic used for TB treatment, with less side effects than isoniazid.
