Alacepril is an angiotensin-converting enzyme inhibitor that has been shown to be effective in treating congestive heart failure and hypertension. It inhibits the formation of angiotensin II, which is a hormone that regulates blood pressure and salt balance. Alacepril also inhibits pergolide mesylate, which is a dopamine receptor agonist used in Parkinson's disease. Alacepril has been shown to increase blood pressure in some patients, so it should be monitored closely. The drug binds to the active site of the enzyme renin and blocks its function by forming an intramolecular hydrogen bond with the side chain carboxylate group of Asp297. This binding prevents the formation of an enzyme-substrate complex that is required for converting angiotensin I into angiotensin II.
