Trans-carboxy glimepiride is a prodrug of the sulfonylurea class and is metabolized in vivo to its active form, glimepiride. Trans-carboxy glimepiride has been shown to have a high affinity for the catalytic site of the enzyme dipeptidyl peptidase IV (DPP-IV), which is responsible for the degradation of many peptides that regulate blood sugar levels. This interaction leads to inhibition of DPP-IV, which prevents inactivation of glucagon and other peptides that regulate blood sugar levels. Trans-carboxy glimepiride also interacts with other enzymes involved in gluconeogenesis, such as phosphoenolpyruvate carboxykinase (PEPCK) and glucose 6-phosphatase (G6Pase).
