3a,6a-Dihydroxypregnan-20-one is a metabolite of progesterone that is converted to 6-hydroxyprogesterone by the action of reductases. It can be synthesized in the adrenal glands or it may be derived from progesterone when it is conjugated with glucuronosides. 3a,6a-Dihydroxypregnan-20-one has been shown to inhibit the synthesis of progesterone by suppressing the production of 17α-hydroxyprogesterone and 11β-hydroxyprogesterone. 3a,6a-Dihydroxypregnan-20-one also affects steroid sulfatase activity by binding to this enzyme. This leads to decreased levels of sex steroid hormones (e.g., testosterone) in mammals and an increase in glucocorticoid levels.
