Diltiazem N-oxide is an n-oxide metabolite of diltiazem, a vasorelaxant. It is a synthetic compound that has been found to have reversed-phase high-performance liquid chromatographic and kinetic properties similar to those of its parent drug. Diltiazem N-oxide has been shown to be an antagonist at calcium channels, leading to decreased levels of calcium in the cell and depolarization of the cell membrane. This agent also exhibits antagonistic activities against other potassium channels, such as delayed rectifier potassium channels. Diltiazem N-oxide is excreted in human urine following oral administration. The plasma concentrations for this agent are higher than those for the parent drug, which may be due to its increased lipophilicity.
