Doxepin N-oxide is a metabolite of doxepin. It is an active metabolite with death as the most common adverse effect at high doses. Doxepin N-oxide has been found to be present in plasma, lung tissue, and blood. This substance also accumulates in tissues and fluids that are not easily excreted such as the brain, lungs, spleen, kidneys, and liver. The concentration range of doxepin N-oxide in body fluids is unknown. Doxepin N-oxide has been shown to have subtherapeutic concentrations when ingested or administered by injection.
