Ethyl 4-fluorophenylglyoxylate is a prodrug that is hydrolyzed in vivo to aprepitant, its active form. Aprepitant is an antiemetic drug that is used to prevent nausea and vomiting caused by cancer chemotherapy or radiation therapy. Ethyl 4-fluorophenylglyoxylate has been shown to inhibit the intestinal absorption of 5-HT3 receptor antagonists such as ondansetron and granisetron, which are used to treat nausea and vomiting. Cyclization of ethyl 4-fluorophenylglyoxylate leads to the formation of an intermediate with high antiemetic activity. Hydrogenation of this intermediate gives aprepitant, which has a more potent antiemetic effect than the parent compound.
