Etoposide is a cytotoxic drug that binds to the enzyme topoisomerase II, which is responsible for breaking and rejoining DNA strands during cell division. Etoposide forms a covalent adduct with this enzyme at the breakpoint region, which results in an irreversible inhibition of its function. The inhibition constant (Ki) is approximately 0.1 μM for human cytochrome P450 3A4 (CYP3A4), which is important for drug metabolism in mammals. Etoposide has been shown to be effective against leukemia cells and other hematopoietic cells, including those of the colon, lung, ovary, and prostate. It also has antiviral activity against influenza virus in vitro. Etoposide induces apoptotic cell death by blocking protein synthesis and preventing activation of transcription factors such as NF-κB and AP-1.
