Fexofenadine is a histamine antagonist that is used to treat the symptoms of allergic rhinitis. This drug is a racemic mixture of two enantiomers, (R)- and (S)-fexofenadine. The drug is well absorbed from the gastrointestinal tract and has a high volume of distribution. It undergoes extensive metabolism by cytochrome P450 enzymes and other enzymes in the liver before it is excreted in urine as metabolites. Fexofenadine does not bind to human mdr1 or p-gp, which means it does not inhibit their function, but may inhibit other drug transport proteins such as OATP1B1 and BCRP. Fexofenadine has been shown to cause disease activity in patients with Crohn's disease or ulcerative colitis when taken at higher doses, which may be due to inhibition of these transport proteins.
