2-Hydroxy-2-methylbutyric acid (2M2B) is a fatty acid that is formed during the metabolism of 2-hydroxymethylglutarate. It has been shown to be present in urine samples from HIV infected patients, and can be detected by NMR spectra and plasma mass spectrometry. The deuterium isotope can be used as a marker for 2M2B in water samples from humans. 2M2B has been found to play an important role in bowel disease, and may be related to mineralization due to its ability to form tiglic acid. The chemical structure of 2M2B is similar to that of hydroxybutyrate, with a hydroxyl group on carbon number two instead of one.
